Anyone who lives, works or volunteers in Hillingdon borough is eligible to be nominated in one of five categories:

Outstanding Young Person,

Star Carer,

Courageous Citizen,

Fabulous Fundraiser

 Community Champion

* An overall Hero of Hillingdon award will be given to the category winner whom the judges feel most deserves the accolade

* If you know someone who deserves credit for doing a good deed, nominate them by filling in the form on this page or online at Use a separate sheet of A4 paper if necessary

* The closing date for entries is Monday, April 29 at 5.30pm and the judges will make their selections shortly afterwards. The judges’ decision at shortlisting and awards stages is final

* Shortlisted nominees will be invited to a gala evening to be held at the Hayes Community Campus in Coldharbour Lane, Hayes on Thursday, June 6. We will only accept nominations on original forms

* Each category winner will receive a trophy and £200 cash, while the Hero of Hillingdon will win a further £500 and trophy. Runners-up will each receive £50

* The judges will be the serving Mayor of Hillingdon for 2013-14, who will be confirmed in post shortly, and the principal of Uxbridge College, Laraine Smith

* Staff and students from Uxbridge College and Trinity Mirror employees cannot be nominated, although they are welcome to nominate other people
