A court hearing has taken place following the death of a woman who was killed by a falling window three years ago.

Four people and three companies were summonsed to appear at Westminster Magistrates’ Court on Wednesday (September 9) to face manslaughter charges, following the death of Amanda Telfer on August 30 2012.

She died when she was struck by a window which fell as she walked along Hanover Square, off Oxford Street, Westminster.

Those summonsed to court and their charges are:

  • Damian Lakin-Hall, 48, Portsmouth Road, Cobham, Surrey: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Claire Gordon, 35, Ashby Crescent, Leeds: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Kelvin Adsett (aka Kelvin Schultz), 63, New Road, Slough, Berkshire: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Steve Rogers, 61, Sheering Mill Lane, Sawbridgeworth, Hertfordshire: summonsed for S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • I S Europe Ltd of Slough, Berkshire: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.
  • Westgreen Construction Ltd, Richmond: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.
  • Drawn Metal Ltd of Leeds, West Yorkshire: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.

The court appearances follow an investigation into Ms Telfer’s death by the Met’s Homicide and Major Crime Command and the Health & Safety Executive.

All seven cases were adjourned to Southwark Crown Court on September 23. The four individuals were granted bail.
