Four individuals and three companies have been told to appear in court next week in connection with the death of a woman who was killed by a falling window in Westminster.

Amanda Telfer suffered fatal injuries three years ago this week, police said, when she was struck by the window as she walked along Hanover Square, off Oxford Street.

The Met said manslaughter charges will be brought against three of the four individuals who have been summonsed to appear, along with the companies, at Westminster Magistrates’ Court on September 9.

The court appearances follow an investigation into Ms Telfer’s death by the Met’s Homicide and Major Crime Command and the Health & Safety Executive.

Those summonsed to court and their charges are:

  • Damian Lakin-Hall, 48, Portsmouth Road, Cobham, Surrey: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Claire Gordon, 35, Ashby Crescent, Leeds: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Kelvin Adsett (aka Kelvin Schultz), 63, New Road, Slough, Berkshire: summonsed for manslaughter by gross negligence and S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • Steve Rogers, 61, Sheering Mill Lane, Sawbridgeworth, Hertfordshire: summonsed for S7(a) Health & Safety at Work Act 1974: Employee breaching general duty at work.
  • I S Europe Ltd of Slough, Berkshire: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.
  • Westgreen Construction Ltd, Richmond: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.
  • Drawn Metal Ltd of Leeds, West Yorkshire: summonsed under S2 Health & Safety at Work Act 1974: Employer breaching general duty of care to an employee and S3 Health & Safety at Work Act 1974: Employer or self employed person breaching general duty of care to non-employee.

Ms Telfer was struck by the falling window on August 30 2012.
