People recovering from a serious illness or injury can get support from a new team in Hounslow.
Health and social care experts will work together to help adults recover and regain their independence following a serious illness, injury or change in life circumstances - including neurological conditions such as a stroke, multiple sclerosis, or Parkinson's.
The new joined-up service will run seven days a week, 365 days a year, from 8am to 8pm, with care available in patients' homes or in convenient community locations.
Councillor Lily Bath, Hounslow Council's cabinet member for adult social care and health, said: "We're really thrilled to have this new integrated service for our residents. It's great that we can now offer help and support to those who need it in their own homes or a community setting, rather than having to go to hospital.
"The work we've done to integrate social care and health services means it's now much easier to access services across both the council and the NHS."
